Reactive Arthritis

Summary

  • Triad of conjunctivitis, urethritis, and arthritis. Usually following urogenital or gastrointestinal infection.
  • Pain associated with mild to moderate swelling and tenderness without significant erythema.
  • Enthesopathy, heel pain and bone spurs. Spondylitis and sacroiliitis can be present.
  • Symptoms can persist for 12 months.

Diagnostic tips

  • Check Urinary tract and eye symptoms.

Tests and Imaging

  • ESR
  • Test urine and stool samples, and possibly synovial fluid
  • Xray
  • Genetic test for HLA-B27

Immediate Treatment

  • Antibiotics
  • Anti-inflammatories
  • Immunosuppressants

Possible Referral

  • Podiatry for management of foot pain, splinting/orthotics, footwear advice and strengthening exercises.
